Clarity encourages clients and families to look over the Welcome Packet prior to arriving to the assessment. Please feel free to print and bring the completed forms to the intake session, however all documentation is offered through your portal on Therapy Appointment 2.0 and is 100% electronic.
Clarity offers electronic documentation through our medical billing and scheduling system, Therapy Appointment 2.0.
To set up an appointment with a provider, all you will need to do is go to TherapyAppointment.com and click on the top right corner where it says "Find Therapist." Type in the desired therapists name and from there you will be asked to create a free username and password through the 2.0 system, if your provider has not already done so. After you complete that, you will have access to your therapists schedule and can complete all new client forms and consents, complete the biography, email your therapist directly, and also request forms to submit to insurance for reimbursement.
Creating or Accessing Your Therapy Appointment Account:
1. Go to www.therapyappointment.com
2. Login Client/Patient Login
3. Therapy Appointment 2.0 Therapist Search
4. Search by name – Therapist's Name (ex: "Alicia Ayvas")
a. Username: Create a username
b. Password: Create a password
6. Complete electronic welcome packet forms, consent forms, and intake paperwork that is all located in your portal
*All active clients have an account, so please DO NOT set up a new account. Simply contact your provider and he/she will email you a link to update your password.
